Free (our fee is paid by the chosen community)
High Country Senior Advisors works with senior living communities across Flagstaff — from large Assisted Living and Memory Care communities to small residential care homes. Our placement services are completely free to families because we are paid by the community, not by you. That means our only agenda is finding the right fit for your loved one. We personally accompany you on tours, help you ask the right questions, and make sure you have everything you need to choose with confidence.
Transitional Care Consulting & Remote Training
$125 / One hour minimum
Hospital discharge arrives faster than anyone expects. We help families make sense of the options, build a realistic plan, and ensure the transition home — or to a new community — is built around your loved one, not around a deadline.

For families who want to keep a loved one at home safely. We visit your home, walk through safety and fall risks using the CDC STEADI framework, and coach caregivers on the practical day-to-day essentials — transfers, routines, dementia communication, and what to watch for before it becomes a crisis. You receive a written summary within 48 hours, with clear action steps and referrals to the right professionals for anything outside our educational scope.
For families managing from a distance, all coaching sessions are also available by phone or Zoom.
